What Doctors Don’t Want You to Know About FUE
Follicular Unit Extraction (FUE) has been in vogue with hair transplantation surgeons in recent years. It is an elegant approach where one follicular unit is taken from a patient’s donor area, one at a time. There are no scalpels or the traditional linear scar. An FUE procedure requires a special 0.7 to 1 mm diameter tool that is used to harvest each hair follicle. All incisions and cuts leave a scar, but an FUE scar is barely visible to the naked eye because each FUE scar shrinks to less than 0.5mm.
There are many variables that contribute to the success or failure of an FUE. As one can imagine, harvesting a single hair follicle one at a time can be extremely tedious and fatiguing to the surgeon if he/she had to do it several hundred times for each procedure. More importantly, negotiating a 0.7 to 1 mm diameter tool to perfectly encompass a hair follicle is technically challenging even under magnification and requires a very steady hand and much experience doing it. Of course, one might ask how a doctor gets that experience and what the cost is to the patients from which that experience comes from.
There are also uncontrollable patient dependent physiologic variables as well, including:
- Hair characteristics such as color and thickness
- Skin characteristics such as hydration level, elasticity, degree of fatty tissue content
- Idiopathic variables (the unknown)
All the above variables contribute to what is called a transection rate. A transection of a hair follicle means that a portion or even the entire hair follicle was cut along its body and could be damaged, which may jeopardize its viability. A complete transected hair follicle will not grow hair when it is implanted.
A “successful†extraction of one hair follicle with the current FUE technique is a very relative term. To better illustrate this point we must understand the anatomy of a follicular unit with respect to the transection rate. One follicular unit can be a group of one, two, three, or four hairs. One patient may have a predominance of two-hair-grouped follicles and the other four-hair-grouped follicles. For example, when a surgeon extracts a four-hair-grouped follicle with an FUE technique and transects half the follicle, only two hairs will grow and the remaining two may be killed off, lost in never-never land. The way some doctors count, this is widely considered a successful FUE effort (not by me, of course), because this means only 50% of hair was harvested and 50% is lost forever! To make matters worse, the patients may be fully charged ($$) for that follicle even with the transection as long a one hair is viable (a shady process to say the least). What is even worse than that is that in a complete transection, that follicle is likely dead forever and even if the doctor did not charge for the complete transaction (as he/she should not charge for it) it would be considered by me to be negative value, reducing the person’s donor hair forever. One may argue that acceptable transection rate for a “successful†FUE is 10% or less, but this is not advertised and most patients (the consumers) do not have a clear understanding of this fact. Nothing in real life is 100%. Even the traditional hair transplant surgery with the donor strip incision has a 2 to 5% transection rate. From a historical point of view, it is interesting to note that New Hair Institute (NHI) was well aware of the possibility of transection rates in excess of 10% as early as 1997 before FUE was in vogue and four years prior to the landmark article published by Rassman, et. al. In conjunction with Dr. Jae Pak (with his mechanical engineering background) the two designed and built a prototype computerized video Follicular Extraction (FLEX) device which was patented by Dr. William Rassman (U.S. Patent 6,572,625). Even that device did not achieve did not consistently achieve the ideal ‘less than 10% transection’ in all patients and FUE can not match the 2 to 5% transection rate of the traditional donor strip incision. The transection rate by our NHI surgeons are still well within the 10% range for FUE and we make no pretenses that it is better than that routinely. Because of inherent limitations of FUE and uncontrollable patient variations, any claims of transection rate of less than 10% should be viewed as highly suspect. The automated process covered by the patent technology (above) is not commercially available… yet!
With the current state of technology, a surgeon may perfect his FUE technique, but the inherent patient variability will keep the FUE transection rate higher than the traditional donor strip incision technique. An informed patient should know the risks and benefits of any surgical procedure. The FUE procedure with its virtually non detectable scarring is an attractive alternative to the traditional donor strip incision and may be good when the amount of hair needed is small because the balding area being treated is not great, but its inherent transection potential may be a deterrent for the very bald patients who want the most hair possible from their donor site.

Just had to comment on the above question re: the non doctor who allegedly has “trained” many doctors in FUE? ARE YOU KIDDING???!!! Would you let someone like that touch your head and put how you look FOR THE REST OF YOUR LIFE in their hands? Please…put down the bong and back away slowly!!
Larry Leonard has no idea what he is doing in terms of FUE or FIT. He barely got the concept before he quit our clinic. He has not been doing anything for ten years and has not trained many doctors. He is a fake.

Thats a real nice article giving the true factors of the FUE. My boyfriend wants to go for this technique and so am here checking out the cons of this and i agree to what you said …its highly unpredictable totally depending on the doctor’s self imagination and experience in magnifying things.

i worked with larry leonard for a year. he trained me and i must say that any sane person would not let him touch them.
the man is full of bologna and owes a lot of people refunds. he is unethical and there are many illegal things going on in that clinic. american hair institute changed to global hair institute look it up!!!
After reading comments about Larry Leonard, I was very surprised. I had a treatment from him when he was working under a doctor with “the vein Clinic”. It was the older procedure were they took a donor strip and disected it for transplants. I will have to say that I was very pleased with the results. The only thing I didn’t like was the stitching job the doctor did. Larry and his crew were great.
As far as the new procedure, I have no experience with him, but would not hesitate to let him do it.
I recently had Larry Leonard perform the FUE procedure on me. It was over 1000 grafts. I was very pleased with his professionlism and knowledge. He basically worked nonstop for over 5 hours in the donor area. A well known docter made the recipient incisions, and Larry and another technician placed the grafts. I had no problem with the healing of either area. I had the hidden transplant, which was nice to hide the donor area after it was done. With any transplant, it takes up to a year for all the hair to fully grow back so I’ve got a long wall till then. I’ll post as the results come along. The previous posts seem old, so maybe Larry has fine tuned his skills.
I’m a black male and I’ve had a few conversations with Larry. With my hair type I will never consider having a strip done. I’ve spoken to three transplanters and Larry seems to be the best by far. The other two were reluctant to tell me the size tool that they use, and couldn’t furnish pictures of African American men like Larry would. If anyone who reads this has any advice on this, has actually known/met Larry, has seen has work, please give me some insight and recommendations. Any insight would be greatly appreciated, but please only complete if you have first hand knowledge, not a grudge to settle. I’m looking into doing this un-reversible procedure soon, and I’m expecting perfection not a crappy job. FYI Larry told me that he only uses the .8mm extraction tools.

before you use him for any of your hair transplant needs, you should know what kind of businessman he is – I just recently was contracted by his office to build him a website, which I did. After going back and fourth several weeks I finished the site and was asked to post it live. After doing so I was told they were not going to pay me for my service because “it didn’t match their vision” – Now mind you they could have said something to that effect 2 weeks after starteing the site instead they let me build and build until the very end and then stiffed me. I intend to persue them in court. Thats just bad business. Know that if you have a problem with your hair transplant – and you don’t like the results, so still have to pay them, but when they don’t like results they stiff their vendors. Seems a bit “One Way” to me….

I met larry and had him put in 1800 grafts…he was incredible, informative and tedious when he worked on me…I’ve called him and he he has called me back..he did exactly what I asked and it looks great..I have huge scars from previous work and he explained how we could take care of them….I will never let anyone but Larry touch me again….TRUST ME, I WAS BUTCHERED FROM PREVIOUS DOCTORS… not one scar from Larry…he worked on me for over 9 straight hours and was awesome….thanks again Larry. I had some work also done by Larry Leonard when he was in Jacksonville, fl and I would have to say that he gave me my life back after a transplant doctor for Jacksonville. Hair restoration did strips and scalp reduction that now scars for life. Larry did a great job of fixing the crop job of what JHR hair transplant doctor did. He is incredible, informative and I would let him work on me again. Better than any hair transplant doc I know.
I had a session of grafts done with Larry Leonard while he was still working as the main assistant for a Dr. in Jacksonville, Florida who ran The Vein Clinic. The Doctor had become an official DHL affiliate location. While the Dr. made the recipient sites with a small gauge needle, Larry and his brother Leif I believe was his name did did all of the work. I actually came to my home for after patient care and helped treat the donor and recipient sites. I was very pleased with the overall procedure and would recommend Larry to anyone interested in having their hair loss taken care of.

Larry did 2000 grafts over two days for me in November of 2008 in a clinic in Redlands, CA. I’m a RN with long and varied experience. The MD was on site and was actually being trained by Larry. In a word….. Larry did a great job! People who knew me bald are blown away. He ‘knew’ to have the recipient sites punched in at an angle so I would have a more natural hair line, and I do. I’m actually going to look him up because he thought I could still donate another 2000 ‘plugs’ and I can fill in and bring my hairline down a cm. Bottom line; did a great job, was informative and had skills I heard him explained to the MD who turned around and gave my wife her eye brows back!
