If you haven’t tried finasteride, it is worth considering as it might reverse some of this thinning. If you add minoxidil the chances are even greater for a benefit without a hair transplant.

The number of grafts to cover the crown depends upon the thickness of your hair (fine, medium or coarse). As the balding area includes the swirl where the hair changes direction, it often takes more grafts to recreate it. I would guess between 1500 grafts (for coarse hair) and 2500 (for fine hair).
Use a mirror and camera together, that is how you do it.

Minoxidil works almost any place you put it and sometimes where you don’t put it as women who use it on their hair find that they may develop increased facial hair. The FDA approved it for the crown and that is why it says that in the insert.
This type of result is not uncommon from finasteride as I see it often.
These double swirls are not common and from a surgeons prospective, when I transplant such a crown I must follow the double swirl to make the hair reconstruction look normal.
